Metronidazole is the gold-standard treatment for trichomoniasis, a prevalent STI caused by the parasite Trichomonas vaginalis. In men, "trich" often presents with minimal symptoms but can cause urethritis (itching or irritation inside the penis, mild discharge, or slight burning after urination or ejaculation). Left untreated, it can increase the risk of transmitting or acquiring other STIs. Metronidazole is also highly effective against other anaerobic bacteria that can cause infections in the prostate and reproductive tract.
Once absorbed, metronidazole penetrates microbial cells and is reduced to its active form. This active compound creates free radicals that damage the DNA of the parasites and anaerobic bacteria, inhibiting their nucleic acid synthesis and ultimately causing cell death. This bactericidal and trichomonacidal action specifically targets the offending pathogens without significantly affecting healthy human cells or aerobic bacteria.
The CDC-recommended regimen for trichomoniasis is a single, 2-gram oral dose (taken as 4 x 500mg tablets at once). Alternatively, a 7-day course of 500mg taken twice daily may be prescribed, which can improve tolerability and efficacy in some cases. It is absolutely crucial to complete the entire course of medication, even if symptoms disappear, to ensure the infection is fully eradicated and to prevent the development of drug resistance.
Avoid all alcohol consumption while taking metronidazole and for at least 72 hours after the last dose. Combining this medication with alcohol can cause a severe disulfiram-like reaction, resulting in intense nausea, vomiting, flushing, headache, and rapid heart rate. Other common side effects include a metallic taste in the mouth, nausea, and darkening of urine (which is harmless).
Trichomoniasis is easily passed between sexual partners. To prevent a frustrating cycle of reinfection, it is essential that all recent partners are treated simultaneously, even if they are not showing symptoms. Individuals should abstain from sexual activity until they and their partners have completed treatment and all symptoms have resolved. Retesting after 3 months is recommended to confirm the infection has been cleared.
